What should we do if a duplicate item appears? (By duplicate, I mean an item we already seen before)

I was thinking of some kind of manually done excel sheet as it's shown in the picture, and maybe we could refine it a bit. (I'd do it of course), but based on how much you guys are commenting, maybe we should look into some sort of Google Forms shenanigans instead? 🤔
